Our program is designed to make family care giving more manageable. This does include a comprehensive approach to care for participants as we make use of specific counseling, physician intervention (visiting doctors) and rehabilitative therapies. Welcome are the elderly, disabled, those with catastrophic injury and the mentally ill.
The Downriver's Nest staff will determine the criteria for admission and whether the adult day activity center is right for their development. The components of the adult day program will include the following:
- Conducting individual needs assessment to determine the person's range of abilities and needs.
- Providing an active program that meets the daily social, recreational, and rehabilitative needs of the person in care.
- Developing an individualized treatment plan for participants and monitoring it regularly, adjusting the plan as necessary.
- Providing referrals to other needed community services.
- Providing a full range of in-house services, which may include personal care, transportation, meal preparation, health screening and monitoring, educational programs, counseling, and rehabilitative services.
- Providing a safe and secure environment.
- Using qualified, well-trained staff and volunteers.
- Adhering to or exceeding existing State and National standards and guidelines.
